My son is 20 months old he wears a size 3 in regular diapers will these fit him
The covers included in the package will fit up to a 35 lb baby. There are 2 sizes of prefolds, the infant size will fit up to a 15 lb baby and the Premium size prefolds will fit up to a 35lbs.

When you say 4x8x4, I think you are specifying the ply or thickness of each diaper, but what are the dimensions of each size?
Yes, the 4x8x4 specifies the layers on the diaper panels. The unwashed dimensions for the Infant size prefolds are 12" x 16" and the Premium size prefolds are 14.5" x 22". With washing they shrink about 10% in length and width.
